This presents difficulty as the microbial makeup of individuals is unique. Further adding to the bacterial complexity, endodontic infections can be characterized as primary, persistent, or secondary. Over 500 microorganisms have been discovered in pulpal infections; however, a core microbiome of 20-30 organisms has been identified. Secondary infections are less diverse than primary and are comprised largely of the following phyla: Enterococcus faecalis, Streptococcus species, Actinomyces species, and Dilaster species. Enterococcus faecalis is unique in being the most consistently found organism in persistent infection.<\/p>\n<p>Even with current root canal disinfection protocols, bacteria persist. In a histological evaluation of teeth with apical periodontitis, biofilms were present in 74% of treated canals. Persistent bacteria may also lead to flare-ups following treatment. Calcium hydroxide, a common intracanal medicagtion, has been found to have a limited effect on E. Faecalis. The use of antibiotics systemically or within an intracanal medicament, has led to an increase in antibiotic resistance. Multidrug resistant E. Faecalis is associated with persistent infections while beta-lactamase resistant gram-negative bacteria have been directly linked to treatment failure of purulent endodontic infections.<\/p>\n<p>Medical research shows that cannabinoids have an antimicrobial effect against gram-positive and gram-negative bacteria. Cannabinoids used in such research include Cannabidiol (CBD), Cannabinol (CBN), and Tetrahydrocannabinol (THC). CBD has\u202fan inhibitory effect on the release of membrane vesicles from gram-negative bacteria impairing bacterial cell communication, thus decreasing antibacterial resistance of biofilms. Cannabinoids added to mouthwash have been proven to have the same antibacterial efficacy as 0.2 % chlorohexidine, lending to its antibacterial potential. In addition to antibacterial benefits, cannabinoids have a positive biocompatible effect on dentin, pulp, and bone regeneration as well as on dental pulp cells.<\/p>\n<p>With limitations of available intracanal medicaments, and the rise of antibacterial resistance, the exploration of novel antimicrobial medicaments is key for future treatment options. The aim of this study was to test the antibacterial efficacy of cannabinoids, CBD, CBN and THC, on bacteria found in persistent endodontic infections; Enterococcusfaecalis (E. faecalis), Streptococcus mutans (S. mutans), and Fusobacterium nucleatum (F. nucleatum).<\/p>\n<p><strong>About the speaker &#8230;<\/strong> Dr. Cassandra Wieczerza lives in Ann Arbor with her husband, two children, and their two golden retrievers! She is from Michigan and met her husband during dental school at the University of Michigan. After graduating in 2016, they moved around the country (Texas and Colorado) for five years while he fulfilled his military commitment. They moved home to Ann Arbor in 2021, where she worked as a general dentist in Plymouth for 2 years before returning to school to complete her graduate endodontic training at the University of Detroit Mercy. She was involved in dental research throughout her dental school and endodontic residency. Her work has gained publication and won first prize at the 2025 Michigan Dental Association. Dr. Wieczerza maintains membership in the American Dental Association, Michigan Dental Association, the American Association of Endodontists, and the Michigan Association of Endodontists. While not in the office, she and her family love to do almost anything outdoors, especially taking nature walks around the many nearby metro parks.
